Why is it not seen in films?

In films, Hrithik’s extra thumb is rarely seen. During shooting, the makers use smart camera angles, hand positions and even VFX to hide it. When a hand is required to be shown, the scene is carefully framed and the final editing is also helpful in hiding it.
